Requesting materials is an impactful way to promote organ, eye, and tissue donation in your community! To track outreach efforts we ask that you fill out a short survey after your event. And please take photos! We always appreciate sharing images of volunteers in action! 

Ambassadors and donation supporters in Oregon and Southwest Washington (Clark, Cowlitz, Skamania, and Walla Walla Counties) can request free educational materials for use at organized personal or community events.  Please submit requests at least three (3) weeks prior to event to ensure timely delivery. If your request is urgent, call our office directly at 503-494-7888.

Please note:

  • If you are hoping to request a Threads of Life Quilt for your event, please fill out the quilt request form instead.
  • Quantity of materials is based on event type and event attendance; all are subject to availability.  
  • Donate Life Northwest serves Oregon and SW Washington (Clark, Cowlitz, Skamania, and Walla Walla Counties); we will only ship materials within our service area. If you live elsewhere, find your state's affiliate by visiting Donate Life America.
