Sign Up on the Donor Registry at DMV

Are you registered as an organ, eye and tissue donor?  Be sure your family knows. The donor code is powerful!  Both Oregon and Washington have passed the Revised Uniform Anatomical Gift Act.

  • In Oregon you have a "D" on your driver's license, permit or ID card.
  • In Washington you have a WA DMV heart on your driver's license, permit or ID card.
  • To join the donor registry, go to a DMV field office in Oregon or Washington or sign up online or contact Donate Life Northwest for a paper registration form. 
  • All ways of registering mean the same thing and are equally powerful.
  • Note: Donor Cards are no longer used in most states, including Oregon and Washington.

About joining the donor registry at DMV:

  • DMV personnel are required by law to ask every person 15 years or older  applying for a driver's permit, new or renewal license, or identification card if they want a "D" or WA DMV heart code signifying their decision to be an organ donor.
  • Parent/guardian consent is not required for individuals 15-17 years old to register as a donor, either at DMV or online.
  • The "D" or WA DMV heart code is considered consent for donation for individuals who are at least 18 years old at their time of death. Parent/legal guardian consent for donation is required at the time of death for potential donors younger than 18 years.
  • The donor code on a driver's license, permit or ID card does NOT include whole body, blood or bone marrow donation.
  • DMV personnel are not expected to answer customer questions about donation/transplantation.  Donate Life Northwest brochures, available at all DMV field offices, provide basic information in both English and Spanish as well as contact phone and email.
  • Currently there is no national donor registry.  Each state has its own individual donor registry. However, the organ, eye and tissue recovery personnel in any given state can call their collegues in another state to ask if an individual is on their donor registry.
