Donate Life Northwest Partner Organization and Volunteer Vaccine Attestation Form

Donate Life Northwest COVID Vaccine Mandate & Safety Protocols:

For the safety of our volunteers and our community, Donate Life Northwest requires all in-person volunteer roles at Donte Life Northwest (DLNW) coordinated events to be fully vaccinated according to the CDC. The purpose of this policy is to mitigate the risk of COVID-19 transmission between DLNW employees, volunteers, and community members. Volunteers who are not fully vaccinated are invited to participate in virtual or remote volunteering opportunities. 

Any person who, in the last 14 days, has: had close contact; experienced symptoms; or tested positive, will not attend Donate Life NW events until they have cleared their return with DLNW staff; return times may vary based on symptoms reported.

Universal and correct mask use will be observed at all times by all persons volunteering/working at Donate Life NW in-person events.

Social distancing of at least six feet will be required between staff, volunteers, and visiting organizations.

Volunteers and Partner Organization staff are asked to attest to full vaccination status by signing this online attestation form or by showing proof of vaccination in-person to a DLNW staff member. Exemptions to the vaccine requirement will be considered on a case-by-case basis. Any approved exemptions will require additional safety measures, including, but not limited to, pre-event COVID-19 testing. 

These requirements are subject to change as the COVID-19 landscape continues to evolve and will be evaluated on an event-by-event basis.
