Forms needed to be printed, signed, & Returned:

• Federal i9 form (2022): pages 1 & 2 must be printed and returned with employee sections completed.
• Federal W4 form (2022): page 1 must be printed and returned with employee sections completed.
• Oregon W4 form (2022): 2022 instructions for Oregon's W4 form can be found here.
• Direct Deposit paperwork: a voided check or equivalent letter from bank must be included with the printed/completed form
• 401K beneficiary designation form
