Being an organ, eye, and tissue donor is more than simply filling out a form. It is a sign of selfless compassion. It is a beacon of hope to those in need. And for the nearly 110,000 children and adults on the transplant waiting list nationally, it is their entire life ahead of them.
In Oregon, and most other areas of the country, you can choose from three ways to register to be an organ, eye and tissue donor:

Register at the DMV
DMV: Register with Permit, License, or ID
Check with your state's DMV for details. In Oregon:
- You must be at least 15 years old to register at DMV.
- By checking the box on your application for a permit, license, or state ID, you are giving legal authorization for organ, eye, and tissue donation upon death, for the sole purpose of saving lives and improving health through transplant.
- Your decision and contact information will be added to the Donor Registry, managed by Donate Life Northwest.
- Registering through the DMV does not include options to exclude specific organs, eyes, or tissue from your decision to donate.
- A heart icon will appear on your card, but is for your information, only. Organ, eye, and tissue recovery agencies check the digital registry and do not rely on a physical ID card. (Paper and online registrants will not have a heart on their cards.)
- If you register through the DMV and then choose to un-enroll as a donor, you will need to purchase a new license, permit or ID through the DMV.

Other states may have similar options. Oregon residents:
- Print a form, here to complete and return by mail.
- You must be at least 13 years old to register.
- By completing and returning the form, you are giving legal authorization for organ, eye, and tissue donation upon death, for the sole purpose of saving lives and improving health through transplant.
- Your decision and contact information will be added to the Donor Registry, managed by Donate Life Northwest.
- If you register in writing through Donate Life Northwest and then choose to un-enroll as a donor, you can contact Donate Life Northwest to do so in writing.
